We work with YOUR doctor, YOUR auto and health insurance, and if need be YOUR attorney. Don’t have a doctor? No problem! We have one on premises! Quality Medical and Physical Therapy offers convenient treatment hours to fit your work schedule; we believe in working with you to resume your usual activities of daily living.
What To Expect & What To Bring With You On Your First Visit:
- Please arrive 30 minutes early for your first appointment to fill out all necessary paperwork
- Referral from your physician (if required)
- Health insurance card
- Driver’s license and other ID
- Employer information (if worker’s comp)
- Claim number for worker’s comp/auto
- Police report for automobile accident
- Automobile insurance information
